Sequoia Integrative Medical Services was founded in 2020 by Dr. Mary Kate Friess DNP, FNP-BC, PMHNP-BC and Jennifer Vanden Heuvel. These visionaries had a dream and a mission of serving our aging population as best they could. Boom — Sequoia was founded, offering in-person and Telehealth medical services in assisted living facilities.
Mary Kate and Jen wanted to offer more comprehensive and individualized care to those aging in place. Many barriers exist that limit aging adults from receiving adequate medical care. Transportation. Resources. Weather. Physical limitations. Even fear.
Sequoia opened its physical headquarters in Kaukauna in 2020 amid the uncertainty of the pandemic. Incredibly, in 5 years, the business has flourished, growing from 0 to over 1700+ active patients and expanding to a service area that spans across Wisconsin and covers 200+ different communities. Much of the success that this business has experienced can be attributed to the dedicated team made up of nurse practitioners, RN’s, and LPN’s, who provide patients and families with peace of mind that medical care doesn’t need to be inconvenient.

Dr. Mary Kate is a board-certified family nurse practitioner with over 30 years of experience in a variety of health care settings. Caring for the underserved and vulnerable is truly one of Mary Kate’s great passions, which is why, after a long career in health care and nursing education, Mary Kate partnered to found Sequoia Integrative Medical Services in 2020. After three successful years, Sequoia continues to positively impact patients in multiple locations.
In her free time, Mary Kate loves being with her two beloved children, her two amazing stepsons, her husband, her family, and her friends. Traveling, hiking, gardening and speaking French creates joie de vivre.

Jen has had the pleasure for over twelve years of working both clinical and administrative in geriatric healthcare, as a caregiver and as various administrative roles. She’s worked in nursing home, assisted living and hospice settings. Jen takes pride in creating and maintaining relationships with our providers, business associates, facility directors, vendors, and all who make Sequoia a functioning entity.
The maturing population is important to her because quality of life should be a priority until we reach our resting place. Jen’s happy place is exploring the ocean’s shelled shores and soaking up the warm weather. Salt life, running, and music hold her heart.

Katelyn is an LPN with over 15 years of experience in healthcare. Over the course of her career, she has transitioned through several moving parts of healthcare. She is dedicated to providing the best care and being the best advocate for her patients.
She has earned the role of Clinical Supervisor where she mentors her peers and continues to promote best nurse practices for patients. Katelyn holds a degree in practical nursing from Northeast Wisconsin Technical College. She lives with her husband and three sons in De Pere.

Stephanie Perra, MSN, RN serves as the Director of Nursing, bringing over 14 years of diverse nursing experience across Long-Term Care, community-based populations, the Emergency Department, and inpatient mental health settings. Her broad clinical background allows her to approach care with a well-rounded, practical, and compassionate perspective.
Stephanie is deeply committed to patient-centered care and believes that exceptional outcomes begin with strong leadership and supportive, positive work environments. She is passionate about mentoring nursing teams, fostering collaboration, and creating systems that empower both caregivers and the patients they serve.
Outside of her professional role, Stephanie is a proud mother of two boys and resides in the Howard area. She enjoys spending time outdoors and unwinding with creative projects and crafts at home. Her balance of professional dedication and personal connection shapes her leadership style — grounded, approachable, and focused on care that truly makes a difference.

I have over 25 years of experience in healthcare. I spent 12 years in the Navy as a Hospital Corpsman, stationed with the Marines and aboard ship, later transferring to an airwing.
I completed my BSN at Chamberlain College of Nursing and my Masters in Healthcare Administration at University of Phoenix. I have been in leadership for the better part of 10 years. I have served as unit manager, Director, and most recently as a CNO before joining the Sequoia team.
As a leader in healthcare, my mission is to lead with integrity and compassion, fostering an environment that prioritizes patient and staff safety while delivering the highest level of care.
I live in DeForest with my husband and our four dogs. In my spare time, I enjoy working in our yard and going on bike rides through the trails in our neighborhood and Token Creek.

Elizabeth comes with a wealth of over 25 years of clinical and healthcare management experience. She has a diverse experience with LTAC, nursing home, assisted living, and hospice. She has successfully managed and help start up businesses. She has a great passion in helping others. She enjoys building relationships and working together to help patients. Elizabeth is a huge advocate for making sure our elderly population gets the care they deserve. Her passion has made her successful in helping helping businesses grow.

I am Ashley, and I am working for Sequoia as the Human Resource Manager. I went to Moraine Park Technical College for my Associate’s Degree in HR and continued on to earn my Bachelor’s Degree in Healthcare Administration from Marian University. I have a passion for all things Human Resources and love to be the support system for employees and also the employer.
Outside of work I am a certified Pilates Instructor and teach regularly at an Athletic Club in Sun Prairie, Wisconsin. I am also a mom to two little girls who keep me very busy. I love to read, go for nice long walks, and bake.

Luella plays a vital role in ensuring smooth operations across multiple departments. With expertise in graphic design, employee onboarding, patient charting, website design, social media management, marketing, and various administrative tasks, she helps keep Sequoia running efficiently.
Her dedication to detail and organizational skills support the team behind the scenes. Whether managing internal processes or enhancing Sequoia’s online presence, Luella’s contributions are instrumental in fostering a well-structured and patient-focused healthcare environment.
Luella enjoys spending time with her husband and their four children, and can be found hiking in Los Angeles, chasing waves in Santa Monica, or camping in Santa Barbara. She is also a Harry Potter expert and avid coffee drinker!

Josie is an accomplished professional in healthcare, with extensive experience across various positions. She possesses a deep understanding of the aging community, enabling her to address the unique challenges and opportunities within this demographic effectively. With a commitment to enhancing the quality of care and fostering innovative solutions, Josie continues to make significant contributions to the field, advocating for the needs and rights of older adults.
In her free time, she enjoys spending time with her husband and 3 daughters, and they love camping, hiking, and spending time in their vegetable garden.


Natalie is a Family Nurse Practitioner who has a passion for caring for her patients and for learning. She started her nursing career as a CNA working in nursing homes around her community. She completed the Accelerated BSN program at UW-Oshkosh. Working in various settings, such as long-term care, hospitals, and in-home health, gave her many opportunities for knowledge and growth as a RN. She obtained her MSN in 2022 through Marian University. She hopes to soon continue on in her education with a certification or doctorate in psychiatric-mental health.
In her free time, Natalie enjoys the fresh air of outside activities, running/exercising, and spending time with her husband and four children.

I have dedicated the past 15 years to working in healthcare, serving in a variety of roles that have strengthened my passion for caring for others. In 2019, I earned my Master’s degree, further building on my commitment to providing compassionate, patient-centered care. At Sequoia Integrative Medical Services, I strive to make every patient feel valued, respected, and supported.
I am married and the proud parent of two children, as well as a grandparent to three wonderful grandbabies who bring so much joy to my life. I live in the small town of Trego, Wisconsin, located in the beautiful northwest part of the state, where I enjoy the peaceful outdoors and close-knit community life.
Outside of work, I enjoy spending time outdoors hunting, fishing, camping, and being on the water. I also have a creative side and enjoy painting and artwork. Whether I’m outdoors with family or working on a creative project, I value the importance of balance, connection, and appreciating life’s simple moments.

Hi, my name is Teal Calaway, and I am a family nurse practitioner. I have been in healthcare since I can remember. I started as a CNA in 2013 and worked my way up to a family nurse practitioner in 2021. I am board-certified by the AANP.
I have always had a passion for helping others, and this job allows me to continue to help others every day. As a family nurse practitioner, it is my goal to help every patient to optimize their physical and mental health to achieve the highest quality of life across all lifespans.
In my personal time, I love spending time with my family and friends. Being a mother and watching my two children grow is my greatest pleasure. We enjoy traveling, exploring, and staying home!

My name is Kayla Guns, and I began my healthcare career as a Certified Nursing Assistant after graduating high school. I earned my Bachelor of Science in Nursing from Bellin College in 2009, worked as a Registered Nurse, and completed my Master of Science in Nursing at the University of Wisconsin-Oshkosh in 2014. As a Nurse Practitioner, I have had the rewarding opportunity to work in various specialties, including family practice, psychiatric care, urgent care, hematology/oncology, and functional medicine.
Outside of work, I cherish spending time with my husband and children, camping in the summer, as well as baking and listening to educational podcasts.

Katie is a board-certified primary care nurse practitioner from Wrightstown, WI. With over 21 years of healthcare experience, she has primarily worked in nursing homes, assisted living facilities, and home health care settings. Katie holds a specialty in wound care and was nationally certified in 2021, having received training in sharps debridement.
Katie has a deep passion for working with the elderly population. She is committed to providing high-quality care based on the most recent evidence-based practice guidelines. Managing multiple chronic conditions can be complex for both patients and their families. Katie develops personalized care plans that prioritize patient well-being and aim to enhance their quality of life.
Outside of her professional responsibilities, Katie enjoys spending time with her two children and immediate family. She is an avid boater and enjoys traveling to the Caribbean.

Hi, I’m Kayla! I am a board-certified Family Nurse Practitioner. After graduating from Alverno College, I started my nursing career and spent several years at Froedtert Health, working on the surgical oncology floor and float pool. Those experiences gave me the opportunity to care for a wide variety of patients and taught me the importance of treating each person as a whole.
I later went on to earn my Master of Science in Nursing through Herzing University. Becoming a nurse practitioner has been such a rewarding path. Helping patients make informed decisions about their health, while focusing on quality of life, is at the heart of my practice.
Beyond my work, travel is a big passion of mine. I have been fortunate to explore many countries already, and I am always excited to keep adding more to the list!



I am a family nurse practitioner. I recently graduated from Concordia University Wisconsin and am board-certified by the ANCC. I am born and raised in the Green Bay, WI area, and have been practicing in health care since 2009 in various roles.
Throughout my career, I have been dedicated to providing patients across the lifespan the right care at the right time, primarily working in skilled nursing facilities, assisted living facilities, and home health. It has become increasingly apparent to me that these three areas of healthcare have some of the highest disparities and I am looking to positively change this.

I am a board-certified Adult Medicine Nurse Practitioner with over 30 years of experience in healthcare, the majority of which has been hospital-based. I earned my Nurse Practitioner degree in 2010 and bring a strong clinical foundation to my practice. As I transition into primary care, my goal is to care for the whole person, with an emphasis on disease prevention, health education, and long-term wellness rather than reactive care alone. I am passionate about partnering with patients to help them achieve their healthiest lives.
Outside of work, I enjoy traveling the world and spending quality time with my husband and three children.

Hello! My name is Pam and I have been a registered nurse specializing in geriatric and mental health since 2010. I was fortunate enough to be Sequoia’s first nurse, and so proud of the growth, success, and team that is now in place. I love my job and my work family…along with all the patients, families, and facility partners I come in contact with. Like they say… It is truly not a job if you love what you do.
In my non-working hours, I prefer to be a homebody or spend time with my two grandchildren, Beckham and Chandler. I live in the Appleton area and would love to get to know you and your families!


I have been a geriatric-based nurse most of my career working in hospice, long term care, dementia, and rehabilitation. Through my work experience, I have loved giving quality nursing care while developing relationships with patients and families.
Outside of work, my favorite thing to do is go camping with my husband, three children, and two rescue dogs. We also spend time at our cottage in Eagle River. Around the house, I always have some DIY project going on.

With more than 20 years of healthcare experience, Ellyn has dedicated her career to providing compassionate, patient-centered care. She began her healthcare journey with over 18 years in various aspects of dental health, where she developed a strong foundation in patient care and education before returning to school to earn her Registered Nurse (RN) degree in 2023. Following graduation, she worked for nearly a year in the hospital medical/surgical setting prior to the hospital’s closure and then spent the last two years working as a Hospice RN, a role that remains especially meaningful to her because of the opportunity to provide comfort, dignity, and support to patients and families during vulnerable times.
Her passion for caring for the aging population led her to join the Sequoia team, whose mission aligns closely with her goal of bringing accessible, personalized healthcare to the community.
Outside of work, she enjoys spending time with her two adult children, family and friends. Her hobbies include fishing, hunting, or simply relaxing on the houseboat with her significant other on the Mississippi River, as well as ice fishing and traveling during the winter months.

With over 20 years of dedicated service in healthcare, Danea brings a wealth of experience and compassion to every role. Throughout a dynamic career, Danea has excelled as a CNA, Med Tech, LPN, and RN, working across diverse settings including long-term care, managed memory care units, MDS, dialysis, utilization management, and case management. This comprehensive background in holistic care coordination and patient assessment aligns perfectly with the mission of Sequoia, supporting seniors to age in place and ensuring tailored clinical experiences in Assisted Living Facilities.
Driven by a passion for helping others, Danea looks forward to further growth with Sequoia & the patients they serve. Outside of work, Danea enjoys time with family, including her husband & two sons. She finds joy in DIY projects, outdoor adventures, long walks with her beloved dog, & travel. Danea is also proud to be part of a large, lively family and cherishes the honor of being both an aunt & a great aunt, as well as sharing adventures and laughter with a close-knit group of friends.


Hi, I’m Kristen a Registered Nurse with 8+ years of healthcare experience. I graduated from Bellin College in 2018. I have worked as a CNA, phlebotomist, RN on cardiology floor, home health and hospice. Helping others and being their advocate is a passion of mine.
I am a dedicated and compassionate nurse who truly values building meaningful connections with my patients. I love being part of their care journey and take pride in providing support, comfort, and attentive care tailored to each individual’s needs. I look forward to meeting new patients, getting to know their stories, and working alongside the provider to ensure they receive the highest quality care. Being able to make a positive impact, even in small ways, is what makes my role so rewarding.
When I am not working I enjoy spending time with family, adventuring with friends, traveling, playing volleyball, finding new coffee spots and staying active.

Paige has been in the medical field since she was 16. She earned her nursing degree from Northeastern Wisconsin Technical College. Her role includes daily triaging of patient needs, monitoring treatment, and providing patient medical care. Believing we all need an advocate in life and in health, Paige does her job with great care and attention. She has specific experience with dementia and Alzheimer’s, as well as a passion for psychiatric care/mental health for all ages.
Paige spends her free time roller-skating on the state trails and acrobatic strength training. She uses her passion for science and health to further improve her body and mind. She is also a natural comedian and always provides a refreshing reminder of just how important it is to laugh!


Hi, my name is Rebecca “Becky” Oliver, and I have worked in healthcare for over 20 years. I started my career as a CNA, later became a Medical Assistant, and then earned my RN license. Throughout the years, I have worked in long-term care, acute care, medical offices, utilization management, and case management.
I have truly enjoyed learning about the many different areas of healthcare and contributing to patient care in a variety of settings. One of the most rewarding parts of my work at Sequoia is building strong relationships with patients and their families while providing compassionate, patient-centered care.
Outside of work, I enjoy spending time at home with my husband and our three cats. I also love reading, watching sports, and working in my garden.

I have been in the healthcare field for 19 years. I was born and raised in Kenosha County, Wisconsin, and I moved to the Green Bay Area 13 years ago to pursue my nursing dreams. I graduated from NWTC in 2015 as an LPN.
Starting at a very early age, I’ve had a burning desire to help others. I became a CNA as soon as I could. I spent my younger years working in group homes, TBI hospitals, and in home health. Hi, my name is Melinda. I have been an RN since 2006. I began my career in healthcare as a senior in high school with completion of my CNA certification. I proceeded to obtain my ADN at Milwaukee Area Technical College followed by my BSN through Chamberlain University. My professional experiences span from bedside care to leadership roles. I take a holistic, patient-centered approach to providing care. My professional experiences include adult and geriatric populations in a variety of settings with acute and chronic conditions, TBI, inpatient mental health services and hospice care. Providing patient and family education to assist them in best practice management of their health is my passion.
Outside of my professional life I enjoy spending time with friends and family, traveling, time in the gym and time out in nature.

Sadie is a Remote Patient Monitoring (RPM) Specialist with an Associate’s degree in Occupational Therapy Assistant and prior experience as a Certified Nursing Assistant. She brings a background in executive-level corporate communications and project management, supporting a thoughtful, collaborative approach to patient care. Sadie is passionate about using technology to support holistic, patient-centered wellness and improve health outcomes.
Outside of work, she maintains a daily yoga practice and enjoys spending time outdoors, traveling, paddle boarding, relaxing at the beach, savoring a good cup of tea, and supporting arts initiatives that foster creativity and connection within the Sheboygan community.



Hello, my name is Mae Lou. I have been working in the medical field for the past 20+ years. I received my medical training from FVTC. I have a degree in EMR First Responder, Advanced Emergency Medical Technician, Nursing Assistant, and AHA CPR Instructor.
I have 20 years’ experience as a medical assistant, 23 years as a CPR instructor, 8 years’ experience working in the Emergency Department, and 24 years teaching at FVTC as EMS Adjunct Faculty. I enjoy giving good quality medical care while working as part of a team to our patients.
I love spending time with my family and friends. I have two adult boys, a wonderful daughter-in-law, and two beautiful grandsons who are my world and my greatest blessing. I enjoy camping, kayaking, Packer games, riding our E-bikes, listening to live music, and hanging out at the family cottage on the Chain O’Lakes. I have a strong faith and God has blessed me.
