EMPLOYEE enrollment steps

STEP 1: SIGN YOUR EMPLOYEE AGREEMENT.

Watch your email for your Employee Agreement, coming from Luella Krautkramer, our Administrative Project Manager.  Review it, print and sign the signature page, scan or take a picture, and sent it back to Luella.

STEP 2: SIGN THE EMPLOYEE HANDBOOK

Print the appropriate handbook for you, and sign both the Employee Handbook Acknowledgement and Receipt page AND the Employee Acknowledgement and Receipt of Harassment Policy Page.  Scan or take a picture, and sent it back to Luella.

STEP 3: bio, Headshot, & Birthday

Biography: Please write a brief biography for yourself so you can be added to the Sequoia website.  Check out the examples on our About page.  

Headshot: If you have a professional headshot already, send along the file(s) to Luella.  If not, please take a headshot yourself following these guidelines:

  • Lighting: Natural lighting is best, but not direct sunlight.  Avoid harsh shadows.
  • Background: A plain background is best, but this can be edited, so don’t worry if you do not have a plain white wall available.
  • Position: Try to get a couple angles; one where your shoulders are square with the camera, and one where your shoulders are at a 45 degree angle, but your face is turned square with the camera.  Be sure to get a shot from your chest up.
  • Camera angle: Be straight on, not angling down or up at your face.
  • Clothing: A solid dark color is best, or any Sequoia apparel.
  • Expression: Remember to smile!
  • Crop: Do not cut off the top of your head.

Birthday: We like to celebrate you on your birthday!  Send your birthday to Luella as well.
STEP 4: SEQUOIA APPAREL

Choose your Sequoia branded clothes.  Sequoia will purchase either the jacket or the sweatshirt for you and another shirt of your choice, OR two shirts for you if you don’t want the sweatshirt.  Send your apparel choices and your size to Luella, along with your shipping address.

STEP 5: SEQUOIA NAME TAG

Your Sequoia name tag will come with your name and your title below.  Send your title to Luella.  For example: 

  • Mary Kate, DNP, FNP-BC
  • Page, RN
  • Heather, LPN
 
STEP 6: SEQUOIA WHatsapp

Join the Sequoia Whatsapp thread to stay in the loop on any team communications.

STEP 7: Accept your invite from Quickbooks.

Look at your inbox for an email from “Sequoia Integrative Medical Services via Workforce (Via Intuit services)” with the subject line, “Your invite to get set up in QuickBooks Workforce.”

Accept the invite so you are able to enter your bank info for direct deposit, your tax documents (I-9, WT-4, and W-4).  You can also view pay details and personal information here.

STEP 8: CREDENTIALING*

1099 EMPLOYEE enrollment steps

STEP 1: SIGN YOUR CONFIDENTIALITY AGREEMENT

Read and sign the Sequoia Confidentiality Agreement.  Scan it or take a picture, and sent it back to Luella.

STEP 2: Accept your invite from Quickbooks.

Look at your inbox for an email from “Sequoia Integrative Medical Services via Workforce (Via Intuit services)” with the subject line, “Your invite to get set up in QuickBooks Workforce.”

Accept the invite so you are able to enter your bank info for direct deposit, your tax documents (W-9).  You can also view pay details and personal information here.
