Our History

OUR HISTORY

Sequoia Integrative Medical Services was founded in 2020 by Dr. Mary Kate Friess DNP, FNP-BC, PMHNP-BC and Jennifer Vanden Heuvel.  These visionaries had a dream and a mission of serving our aging population as best they could.  Boom — Sequoia was founded, offering in-person and Telehealth medical services in assisted living facilities.

Mary Kate and Jen wanted to offer more comprehensive and individualized care to those aging in place. Many barriers exist that limit aging adults from receiving adequate medical care.  Transportation. Resources. Weather. Physical limitations. Even fear.

Sequoia opened its physical headquarters in Kaukauna in 2020 amid the uncertainty of the pandemic. Incredibly, in 5 years, the business has flourished, growing from 0 to over 1400+ active patients and expanding to a service area that spans across Wisconsin and covers 180+ different communities.  Much of the success that this business has experienced can be attributed to the dedicated team made up of nurse practitioners, RN’s, and LPN’s, who provide patients and families with peace of mind that medical care doesn’t need to be inconvenient.

MARY KATE FRIESS

Dr. Mary Kate Friess DNP, FNP-BC​, PMHNP-BC, Director of Clinical Operations & Co-Owner

Dr. Mary Kate is a board-certified family nurse practitioner with over 30 years of experience in a variety of health care settings. Caring for the underserved and vulnerable is truly one of Mary Kate’s great passions, which is why, after a long career in health care and nursing education, Mary Kate partnered to found Sequoia Integrative Medical Services in 2020. After three successful years, Sequoia continues to positively impact patients in multiple locations.

In her free time, Mary Kate loves being with her two beloved children, her family and her friends. Traveling, hiking, gardening and speaking French creates joie de vivre.

JEN VANDENHUEVEL

Jennifer Vanden Heuvel, Director of Admissions & Co-Owner

Jen has had the pleasure for over twelve years of working both clinical and administrative in geriatric healthcare, as a caregiver and as various administrative roles. She’s worked in nursing home, assisted living and hospice settings. Jen takes pride in creating and maintaining relationships with our providers, business associates, facility directors, vendors, and all who make Sequoia a functioning entity. 

The maturing population is important to her because quality of life should be a priority until we reach our resting place. Jen’s happy place is exploring the ocean’s shelled shores and soaking up the warm weather. Salt life, running, and music hold her heart.

KATELYN WERY

Katelyn Wery, LPN​, Clinical Supervisor

Katelyn is an LPN with over 15 years of experience in healthcare.  over the course of her career, she has transitioned through several moving parts of healthcare.  She is dedicated to providing the best care and being the best advocate for her patients.

She has earned the role of Clinical Supervisor where she mentors her peers and continues to promote best nurse practices for patients.  Katelyn holds a degree in practical nursing from Northeast Wisconsin Technical College.  She lives with her husband and three sons in De Pere.

  
ELIZABETH ATKINSON

Elizabeth Atkinson, Regional Director of Business Development

Elizabeth comes with a wealth of over 25 years of clinical and healthcare management experience. She has a diverse experience with LTAC, nursing home, assisted living, and hospice.  She has  successfully managed and help start up businesses.  She has a great passion in helping others.  She enjoys building relationships and working together to help patients. Elizabeth is a huge advocate for making sure our elderly population gets the care they deserve. Her passion has made her successful in helping helping businesses grow. 
 
In her free time, she enjoys spending time with family and friends. 
  
ASHLEY COLKER

Ashley Colker, Human Resources Manager

I am Ashley, and I am working for Sequoia as the Human Resource Manager. I went to Moraine Park Technical College for my Associate’s Degree in HR and continued on to earn my Bachelor’s Degree in Healthcare Administration from Marian University. I have a passion for all things Human Resources and love to be the support system for employees and also the employer.


Outside of work I am a certified Pilates Instructor and teach regularly at an Athletic Club in Sun Prairie, Wisconsin. I am also a mom to two little girls who keep me very busy. I love to read, go for nice long walks, and bake.

  
JESSALYN CONNELL

Jessalyn Connell, RRT, Adminstrator

With a robust career spanning 14 years in healthcare and 9 years as a dedicated Registered Respiratory Therapist, Jessalyn brings a wealth of experience and expertise to patient care. Passionate about championing patient-centric and accessible healthcare, Jessalyn believes that every patient deserves personalized and easily accessible healthcare tailored to their unique needs.
 
Beyond her professional pursuits, Jessalyn enjoys spending quality time with her family, practicing yoga for holistic well-being, and honing her skills in photography. Her diverse interests and dedication to patient advocacy make her an invaluable asset in the healthcare community, embodying a compassionate and patient-centered approach to healthcare delivery.
LUELLA KRAUTKRAMER

Luella Krautkramer, Administrative Project Manager

Luella plays a vital role in ensuring smooth operations across multiple departments. With expertise in graphic design, HR functions, patient charting, website design, and various administrative tasks, she helps keep Sequoia running efficiently.

Her dedication to detail and organizational skills support the team behind the scenes. Whether managing internal processes or enhancing Sequoia’s online presence, Luella’s contributions are instrumental in fostering a well-structured and patient-focused healthcare environment.

Luella enjoys spending time with her husband and their four children, and can be found hiking in Los Angeles, chasing waves in Santa Monica, or camping in Santa Barbara.  Luella is also a Harry Potter expert and avid coffee drinker! 

JOSIE LENZ

Josie Lenz, Admissions Manager

Josie is an accomplished professional in  healthcare, with extensive experience across various positions. She possesses  a deep understanding of the aging community, enabling her to address the unique challenges and opportunities within this demographic effectively. With a commitment to enhancing the quality of care and fostering innovative solutions, Josie continues to make significant contributions to the field, advocating for the needs and rights of older adults.

In her free time, she enjoys spending time with her husband and 3 daughters, and they love camping, hiking, and spending time in their vegetable garden.

NATALIE BANDA

Natalie Banda, APNP, FNP-BC

Natalie is a Family Nurse Practitioner who has a passion for caring for her patients and for learning. She started her nursing career as a CNA working in nursing homes around her community. She completed the Accelerated BSN program at UW-Oshkosh. Working in various settings, such as long-term care, hospitals,  and in-home health, gave her many opportunities for knowledge and growth as a RN. She obtained her MSN in 2022 through Marian University. She hopes to soon continue on in her education with a certification or doctorate in psychiatric-mental health.

In her free time, Natalie enjoys the fresh air of outside activities, running/exercising, and spending time with her husband and four children.

  
DEB BARTMANN

Deb Bartmann, RN MSN-APNP

Deb Bartmann is a Nurse Practitioner with a focus on Adult and Gerontology patients. She started her career in 1989 as a CNA. Deb has been a Registered Nurse since 1991. She completed her BSN in 2008 through University of Phoenix. She obtained her MSN in nursing educations in 2010 through University of Phoenix, and obtained MSN- Nurse practitioner in 2012 through Marian University.
 
In her spare time, she enjoys spending time traveling with her family, gardening, cooking  and baking.
HILLARY BUSCH

Dr. Hillary Busch, DNP, APNP, FNP-BC

My name is Hillary Busch. I worked as a Registered Nurse since 2013 before deciding to pursue my doctorate degree. I earned my Doctorate of Nursing Practice from UW Oshkosh in 2023. Throughout my career, I have worked in trauma, critical care, labor and delivery, hospice, and primary care. My love of many different fields led me to becoming a family nurse practitioner. I have a passion for advocating for vulnerable populations such as those affected by mental illness and addiction. Most recently, I staffed a 300+ bed nursing home for veterans and I found deep satisfaction in caring for the elders in my community. My patients describe me as approachable, empathetic, kind, and thorough. I take the time that is needed to provide the highest level of care to you and your loved ones. I love to laugh with my patients and celebrate joyful moments and achievements. I consider it an honor to be your family nurse practitioner at Sequoia.

My husband and I have two kids, Margo, age 7, and Silas, age 10. We home school our children and run a small homestead with a big garden, ducks and chickens. We love canning, preserving, birding, hunting, fishing and road trips!

TEAL CALAWAY

Teal Calaway, MSN, APNP, FNP-C​

Hi, my name is Teal Calaway, and I am a family nurse practitioner.  I have been in healthcare since I can remember.  I started as a CNA in 2013 and worked my way up to a family nurse practitioner in 2021.  I am board-certified by the AANP.

I have always had a passion for helping others, and this job allows me to continue to help others every day.  As a family nurse practitioner, it is my goal to help every patient to optimize their physical and mental health to achieve the highest quality of life across all lifespans.

In my personal time, I love spending time with my family and friends.  Being a mother and watching my two children grow is my greatest pleasure.  We enjoy traveling, exploring, and staying home!

HALEY DECERO

Haley DeCero, APNP, FNP-BC

Hi, I’m Haley!  I’ve been a nurse for 13 years.  I most recently graduated in 2020 with my masters as a family nurse practitioner. 

The majority of my nursing career has been in various senior living roles and working with the geriatric population. I whole-heartedly enjoy working as a nurse and couldn’t imagine doing anything else.

I enjoy spending time with my family – my husband, our two toddlers, and our dog. I love going out to eat, trying new foods and drinks, and traveling as much as we can.

  
KAYLA GUNS

Kayla Guns, APNP, FNP-BC

My name is Kayla Guns, and I began my healthcare career as a Certified Nursing Assistant after graduating high school. I earned my Bachelor of Science in Nursing from Bellin College in 2009, worked as a Registered Nurse, and completed my Master of Science in Nursing at the University of Wisconsin-Oshkosh in 2014. As a Nurse Practitioner, I have had the rewarding opportunity to work in various specialties, including family practice, psychiatric care, urgent care, hematology/oncology, and functional medicine. 

Outside of work, I cherish spending time with my husband and children, camping in the summer, as well as baking and listening to educational podcasts.

  
Katie Hagerstrom

Katie Hagerstrom, MSN, AGPCNP-BC, WCC

Katie is a board-certified primary care nurse practitioner from Wrightstown, WI. With over 21 years of healthcare experience, she has primarily worked in nursing homes, assisted living facilities, and home health care settings. Katie holds a specialty in wound care and was nationally certified in 2021, having received training in sharps debridement.

Katie has a deep passion for working with the elderly population. She is committed to providing high-quality care based on the most recent evidence-based practice guidelines. Managing multiple chronic conditions can be complex for both patients and their families. Katie develops personalized care plans that prioritize patient well-being and aim to enhance their quality of life.

Outside of her professional responsibilities, Katie enjoys spending time with her two children and immediate family. She is an avid boater and enjoys traveling to the Caribbean.  

DONNA HAWLEY

Dr. Donna Hawley, DNP, APNP, PMHNP, CDP, Certified Dementia Practitioner

Donna J. Hawley, DNP, APNP, PMHNP, CDP, is the founder and program lead of the Sequoia Pathways: Dementia Education & Support Program, launching in 2025. A board-certified Psychiatric Mental Health Nurse Practitioner and Certified Dementia Practitioner and Trainer, Dr. Hawley brings more than 25 years of experience in geriatrics, dementia care, mental health, hospice, and palliative care.

She earned her Doctor of Nursing Practice and Master of Science in Nursing Education from the University of Wisconsin–Oshkosh, and her Psychiatric Nurse Practitioner Certification from the University of Wisconsin–Milwaukee. Alongside her clinical practice, she has been a nursing faculty member at UW–Oshkosh since 2013, teaching and mentoring the next generation of nurses.

Donna is an active committee member with the Alzheimer’s Association. She has also served as an expert representative for the Wisconsin Department of Human Services Long-Term Care Advisory Board.

Her vision is to expand dementia education and support services statewide and beyond, ensuring that caregivers and care teams have the tools they need to deliver compassionate, person-centered care and improve the quality of life for those living with dementia.

  
KAYLA HIMEBAUCH

Kayla Himebauch, MSN, APNP, FNP-BC

Hi, I’m Kayla! I am a board-certified Family Nurse Practitioner. After graduating from Alverno College, I started my nursing career and spent several years at Froedtert Health, working on the surgical oncology floor and float pool. Those experiences gave me the opportunity to care for a wide variety of patients and taught me the importance of treating each person as a whole. 


I later went on to earn my Master of Science in Nursing through Herzing University.  Becoming a nurse practitioner has been such a rewarding path. Helping patients make informed decisions about their health, while focusing on quality of life, is at the heart of my practice. 

Beyond my work, travel is a big passion of mine. I have been fortunate to explore many countries already, and I am always excited to keep adding more to the list!

NICOLE LI

Nicole Li, FNP-C PMHNP-BC

Hi! My name is Nicole. I am a dual certified licensed family nurse practitioner and psychiatric mental health nurse practitioner.  With over 10 years of experience as a registered nurse, I primarily worked with the geriatric population and LTAC settings before pursuing my master’s degree.  I have worked as a family nurse practitioner for almost one year, mostly working with the geriatric population. Currently, my specialty lies in psychiatry, where I have practiced for over 5 years across correctional, outpatient and inpatient hospital settings.
 
My approach to providing care is to be patient-centered. I strive to provide care that is respectful of, and responsive to, individual patient preferences, needs, and values and ensure that patient values guide all clinical decisions.
I hope to continue my education with a doctorate degree in psychiatric mental health.
 
In my free time, I love spending time with our 4 children and our dog, Chewy.   I enjoyed cooking, camping, traveling and canvas painting.
LEANNA MUNOZ

Leanna Munoz, APNP, FNP

Leanna Munoz brings 18 years of dedicated service in healthcare, with 13 years specifically devoted to her role as a Family Nurse Practitioner. She is entrusted with the care of the Eau Claire community and its surrounding areas, where she has made a lasting impact through her commitment to patient well-being and comprehensive healthcare practices.
 
Leanna’s passion lies in providing compassionate and high quality healthcare services. She believes in the power of effective teamwork and collaboration, always striving to ensure that patient interests remain at the forefront of every decision and treatment plan.
 
Outside of her professional responsibilities, Leanna finds joy in spending cherished moments with her spouse and four children. Their shared love for travel and exploration fuels their adventurous spirit, always eagerly planning their next exciting journey. Leanna’s dedication to her patients, coupled with her zest for life and family, makes her a valued healthcare provider and community advocate.
  
ALLISON RUTTEN

Allison Rutten, APNP, FNP-BC

I am a family nurse practitioner. I recently graduated from Concordia University Wisconsin and am board-certified by the ANCC. I am born and raised in the Green Bay, WI area, and have been practicing in health care since 2009 in various roles.

Throughout my career, I have been dedicated to providing patients across the lifespan the right care at the right time, primarily working in skilled nursing facilities, assisted living facilities, and home health. It has become increasingly apparent to me that these three areas of healthcare have some of the highest disparities and I am looking to positively change this.

  
REBECCA TREWYN

Rebecca Lynn Trewyn, BSN, RN, MSN, ANP-BC, PMHNP-BC

Rebecca Trewyn is a board-certified Adult Nurse Practitioner and board-certified Psychiatric Mental Health Nurse Practitioner with over 30 years of experience across a variety of healthcare settings.  Her approach to care emphasizes treating the whole person—recognizing that mental health and physical health are deeply interconnected. She is passionate about serving underserved populations
and is committed to providing compassionate, evidence-based care.

Rebecca’s philosophy of nursing is grounded in caring, listening, validating each person’s experiences, and meeting each client where they are at, while applying sound medical and psychiatric practices. She places a strong emphasis on de-prescribing unnecessary medications whenever possible, instead focusing on the four pillars of health: Nutrition, Sleep, Exercise, Stress Management & Healthy Relationships. 

Through this holistic and patient-centered approach, Rebecca strives to empower individuals to achieve their best possible well-being. She looks forward to the opportunity to meet and care for each person she serves.

PAM LEPLAVY

Pam Leplavy, Lead Clinician RN

Hello! My name is Pam and I have been a registered nurse specializing in geriatric and mental health since 2010. I was fortunate enough to be Sequoia’s first nurse, and so proud of the growth, success, and team that is now in place. I love my job and my work family…along with all the patients, families, and facility partners I come in contact with. Like they say… It is truly not a job if you love what you do.

In my non-working hours, I prefer to be a homebody or spend time with my two grandchildren, Beckham and Chandler. I live in the Appleton area and would love to get to know you and your families!

Hello, I’m Traci!   I’m a dedicated Registered Nurse with over 35 years of experience in healthcare, committed to providing exceptional patient care. Throughout my career, I’ve had the privilege of working across various healthcare settings, gaining extensive expertise in gerontology, orthopedics, oncology, and behavioral health. 
Helping others is my passion. I value building meaningful relationships with my patients while striving to enhance their experience and improve healthcare outcomes.
 
In addition to my professional life, I’m passionate about lifelong learning and making lasting moments. I enjoy traveling, reading, gardening, and cherishing time with my children, grandchildren, and all others around me.
TASHA BRONNER

Tasha Bronner, RN

Hello, my name is Tasha. I started my journey as a healthcare professional in 2006 working as a resident care assistant in an assisted living community before becoming a certified medical assistant in 2010 and working in a pediatric clinic for 8 years. In 2018, I earned my nursing degree and some of the roles I have since served in include inpatient RN, hospice RN Case Manager, and Director of Nursing. Throughout my healthcare career, I have become a strong patient advocate, working to help overcome barriers often faced within the healthcare system. 
 
In my free time, my husband and I are busy with our 2 kids, and we can often times be found on a baseball diamond, soccer field, basketball court, or in a gymnastics facility. When we aren’t at a sporting event, we enjoy the outdoors, hiking, camping, or spending time with our friends and family.
ASHLEY HINES-WILHELM

Ashley Hines-Wilhelm, RN

Hello, I’m Ashley! I have been a registered nurse since 2018 but have been in healthcare for 15+ years. My nursing career started out as a private duty caregiver then progressed to a CNA, LPN, and finally as a RN. I obtained my bachelor’s degree in teaching and history at Silver Lake College. While still working as a CNA, I worked as a middle/high school teacher. I enjoyed teaching but it wasn’t my true calling. I went back to school to become a RN at NWTC.
 
I have worked in manufacturing nursing, cardiology, ENT, primary care, and hospice. I am currently working on obtaining a master’s degree with an emphasis as a nurse practitioner. I love helping people and aim to provide safe and quality care for each patient I meet. I strive to be each patient’s advocate. I am blessed to be a part of the Sequoia team!
 
In my free time, I love spending time with my family, crafting, reading, traveling, or being outside. I am married and have two wonderful children. 
KRISTEN LENSMEYER

Kristen Lensmeyer, RN

Hi, I’m Kristen a Registered Nurse with 8 years of healthcare experience.  I graduated from Bellin College in 2018. I have worked as a CNA, phlebotomist, RN on cardiology floor, home health and hospice.  Helping others and being their advocate is a passion of mine.  

When I am not working I enjoy spending time with family, adventuring with friends, traveling, playing volleyball, finding new coffee spots and staying active. 

TAYLOR LOCKHART

Taylor Lockhart, BSN, RN

Hey everyone! My name is Taylor and I am a Registered Nurse with over 10 years of experience in the healthcare field. I am originally from Elkhorn, WI. I attended Marian University in Fond du Lac, WI to obtain my BSN degree and have remained in Fond du Lac ever since as my significant other owns a restaurant that ties us down quite a bit.


While I was in nursing school, I worked as a medical scribe in the Emergency Department. After graduating in 2018, I transitioned to an RN position in the Emergency Department. I recently left the Emergency Department and am building a healthier work life balance for myself and my family. I am thrilled to be starting with Sequoia as I am eager and passionate about serving our aging population in the best way possible. I am looking forward to a position in which I will be
able to build a relationship with patients and their families as we navigate their care together.


I will be working with Sequoia part time alongside my full time position as a RN in Urgent Care. I have a strong work ethic and loved to keep busy… if I am not at Urgent Care or dedicating time to Sequoia, you will find me helping out at my boyfriend’s restaurant or spending time with our 3 spunky and loving dogs!

PAIGE MAURINA

Paige Maurina, RN

Paige has been in the medical field since she was 16. She earned her nursing degree from Northeastern Wisconsin Technical College. Her role includes daily triaging of patient needs, monitoring treatment, and providing patient medical care. Believing we all need an advocate in life and in health, Paige does her job with great care and attention. She has specific experience with dementia and Alzheimer’s, as well as a passion for psychiatric care/mental health for all ages.

Paige spends her free time roller-skating on the state trails and acrobatic strength training. She uses her passion for science and health to further improve her body and mind. She is also a natural comedian and always provides a refreshing reminder of just how important it is to laugh!

  
EMILY MILLER

Emily Miller, MHA, BSN, RN

Hello, I’m Emily, a registered nurse.  I have worked in many different departments in healthcare including registration, medical records, staffing, payroll, and even volunteering. 
 
I have a Master’s degree in Health Care Administration. I decided I wanted to be a part of caring for the patients. Therefore, I received my CNA, LPN, and then RN. I have worked on medical floors, ERs, ICU, and homecare. I enjoy getting to know those I care for, advocating for them, and empowering them with education. 
 
When not working, you will find me, my amazing husband, our two teenagers, and 2 dogs remodeling our cabin, fishing, hunting, spending time with friends, and eating ice cream. 
  
REBECCA OLIVER

Rebecca Oliver, RN

Hello! My name is Rebecca (Becky) and I have been a Registered Nurse since 2012. I have over 20 years of experience in healthcare.  I began as a CNA, became a Medical Assistant and then an RN. I have worked in LTC, acute care, medical office, utilization management and in case management. Throughout my career it’s been wonderful to learn about different aspects of healthcare and to contribute to patients’ health in a variety of settings.


I am excited to join Sequoia to build strong relationships with patients and their families to provide patient-centered care.


In my non-working time I enjoy being at home with my husband and our 4 cats. I also love to read, watch basketball and spend time in my garden. 

WENDY REISS

Wendy Reiss, RN

With over 15 years of experience in healthcare, Wendy has dedicated her career to providing exceptional care and support to patients and their families. Her deep compassion and empathetic nature create a calming environment for those she works with to feel well-cared for and comfortable.  Wendy’s diverse background spans oncology, IV therapy, trauma, surgical care, and hospice, showcasing her versatility and commitment to improving the lives of others. She is known for going the extra mile to build meaningful and trusting relationships with her patients, ensuring that every individual feels seen, heard, and supported throughout their healthcare journey.

 
On her days off you will most likely find Wendy with her husband cheering on one of their four daughters at various sporting events. If there’s a quiet night she enjoys working on puzzles, a hot cup of tea and great conversation.
 
MORGAN SCHWEITZER

Morgan Schweitzer, BSN, RN

Hi everyone! My name is Morgan, and I am a registered nurse with experience in multiple different aspects of healthcare including cardiovascular health, memory care and dementia, hospice, and ICU experience as well. I received my bachelor’s degree in science of Nursing at Marian University in Fond du Lac, WI in 2018 and have been practicing nursing ever since. 

My goal as a nurse is to promote and provide quality care, as well as be the biggest advocate for my patient population. Providing empathy and compassion along the way, my goal is to make a positive impact and difference on people’s lives. I’m driven with a deep desire to care for others and support them during their most vulnerable moments. I truly love being a nurse, and helping others feel comfort during an uncomfortable time. 
 
When I am not nursing, my favorite things to do are going to Crossfit, golfing, hunting, lounging by the pool, playing outside with or walking my 2 golden retrievers Sloan & Bueller, or going to my favorite vacation spot and enjoying the beach in Cape Cod eating seafood. 
 
I am so excited to be a part of Sequoia and I look forward to working with you and your loved ones. 
  
MACKENZIE VAN ASTEN

Mackenzie Van Asten, RN​

Hi! My name is Mackenzie, and I am a Registered Nurse with over 10 years of experience working in healthcare. 

With a background rooted in a passion for healthcare, my focus is on providing diligent patient care.  Sequoia offers an opportunity for me to apply my skills and contribute to a compassionate healthcare team.  My goal is to build meaningful connections with patients, support their health journeys, and continue to develop professionally in this unique environment.

In my personal time, I love spending time with my two children and fiancé.  You can often find us camping at our happy place in Door County. 
  
ANNA WUCHERER

Anna Wucherer, RN, BSN, Triage Nurse

My name is Anna and I have been an RN since 2023. I have had my CNA license since 2019. I have substantial experience with inpatient psychiatry and psychology as well as experience working on a cardiology/ PCU floor. I obtained my BSN from Chamberlain university in 2025 and am continuing my education with Chamberlain to become a psychiatric nurse practitioner. I have a passion for giving my patients the tools and education to advocate for their wellness and health and strive to build my nursing practice around empathy and understanding.

When not at work, you can find me, my 3 children aged 14, 12, and 10, and my husband to be at all the youth extracurriculars and sporting events imaginable. When our time is not taken over by youth events, we are kayaking, hiking, or camping. I am excited to join this team and to provide top-quality healthcare to you and your family. 
  
HEATHER ALAND

Heather Aland, LPN, Head Triage ​

I have been in the healthcare field for 19 years. I was born and raised in Kenosha County, Wisconsin, and I moved to the Green Bay Area 13 years ago to pursue my nursing dreams. I graduated from NWTC in 2015 as an LPN.

Starting at a very early age, I’ve had a burning desire to help others. I became a CNA as soon as I could. I spent my younger years working in group homes, TBI hospitals, and in home health. Since becoming a LPN, I have focused on our aging population and their quality of life and care. I set out each day looking for opportunities to make someone’s day better.

With Sequoia, we work closely as a team, making each interaction a priority. From Triage, Telehealth and in-person visits, our team is dedicated to you every step of the way.

STEPHANY GRENZER

Stephany Grenzer, LPN Triage​

I have over 22 years of experience working in healthcare as a caregiver and an LPN.  I am dedicated to providing care with compassion and empathy.  My goal is to build new relationships, inspire hope in patients’ healthcare journeys, help families find comfort in our healthcare team, and continue to expand in my career development.  I live by the quote, “When you have a dream, you’ve got to grab it and never let go.”

When not at work, you can find me with my husband of 21 years, my daughter who is 18, and son who is 13.  Much of my time is spent with a basketball and a court of some sort with my children.  My hobbies are camping, hiking, sports, and spending time with family.   
 
 
  
CHRISTINE MEDINA

Christine Medina, RN, BSN Triage Nurse

My name is Christine and I am a Registered Nurse. I obtained my CNA certification as a junior in high school and have been working in healthcare ever since. I became an LPN in 2021. I graduated from the ADN program at Fox Valley Technical College in 2022 and obtained my RN license shortly after. I attended the RN to BSN program and University of Wisconsin Green Bay and graduated with my BSN in May of 2024.  I’m currently in school for my Doctorate in Nursing Practice on the Psychiatric Mental Health Nurse Practitioner track. 
 
In my 10+ years of healthcare experience, I have worked in long-term care, family practice, and behavioral health.  I strive to provide the highest quality of care to my patients and I have a passion for learning. 
 
When I am not working you can often find me spending time with my husband and our black lab. I enjoy good coffee, reading, and spending time with my family.
TAYLOR PERL

Taylor Perl, RPM Coordinator

Hi, I’m Taylor! I am the RPM Coordinator for Sequoia, working behind the scenes to continuously monitor patient health.  I am a recent graduate of Marian University with a Bachelors of Science in Biology degree. I have worked in many different healthcare settings including as a CNA in a long term care facility, a PCT on the neuro medical-surgical inpatient unit, a medical scribe in the ER, and a pediatric home-health caregiver. 
 
When I’m not working, you can find me at a variety of sporting events supporting my four younger siblings, traveling the states, enjoying a good book, or going for a run!
ALICIA DAHLKE

Alicia Dahlke, Geriatric Medical Assistant

Hello, My name is Alicia. I have been a Certified Medical Assistant for 10+ years. I graduated from Rasmussen University and have been dedicated to providing compassionate and quality care to patients as well as supporting some amazing doctors and practitioners ever since. I started my career in OB/GYN then spent 5 years working in Neurosurgery and another 5 years working in Urology. I love what I do and have always had an affinity for caring for people.
 
Outside of work I am a mother of 2 amazing teens, 2 dogs and 2 cats. I love to spend time outdoors camping, kayaking, paddling boarding, disc golfing, hiking and taking nature photos. I have a passion for cooking, live music and taking care of the people I love.
KAYLIA HADLEY

Kay Hadley, Geriatric Medical Assistant

I have been in the medical assisting field for five years, with additional experience in healthcare working in long-term care facilities.  I attended Rasmussen University, where I developed a strong foundation in clinical skills and patient care.  I am committed to delivering quality care and being a dependable support to both patients and healthcare teams.
 
Outside of work, I enjoy spending time with my family.  As a busy mom of three children, family life is very important to me.  Faith plays a meaningful role in our lives and helps us stay grounded.  My family and I enjoy outdoor activities like bike rides and family vacations.  Fall is my favorite season, and I enjoy photography, especially capturing nature’s beauty.  I am passionate about professional growth and finding balance between my career, purpose, and personal values.
