At Sequoia we are dedicated to providing care with our team of physicians, nurse practitioners, nurses and therapists that offer dynamic and multifaceted approaches to patient care. We are dedicated to providing local care which allows our team to be on-call for both patients and facilities needs. We coordinate our care with facilities to provide each patient care that is tailored to their wants and needs.
Sequoia has locations in NorthEast Wisconsin with local staff who are dedicated to serving you at any time. We work with your insurance and healthcare providers to decide what service is best for your and your journey.
Geriatrics
Our staff currently provides medical and psychological care in assisted-living facilities for patients aging-in-place. We provide medical examination, diagnosis, treatment, and ongoing wellness to our patients. We believe that the best version of our patients starts with a healthy body and mind. We believe that addressing both aspects simultaneously allows for the most balanced health and healing process.

Telehealth
We offer tele-health services to patients seeking a virtual visit with a nurse practitioner or therapist. In the future, we will be offering in-person visits at our Kaukauna office, however, for now, please let us help you virtually from the comfort of your own home!

Medical Clearance for Psychiatric Hospitals
We provide medical clearance and ongoing medical support for patients during their treatment at psychiatric facilities. Providing a balance of physical and mental health care is very important to our mission and integrates well with the psychiatric hospitals whom partner with Sequoia.
We look forward to being able to expand our offerings in the future while focusing on the comprehensive health and well-being of those whom we meet.

We work with your facility or independent care home to bring services to you. We also offer telehealth services so you can have consultations from the comfort of your home.
